The tag "Corrupt Form," has been placed to alert you that one (or more) of your uploads is a corrupt image meaning the file is either too large or in a format that is not compatible with Med+Proctor.

PNG files are always compatible and are the preferred format for all document submissions uploaded to Med+Proctor.

Most iPhone or mobile phone photos and screenshots are saved to your device in the pre-set format of an HEIC file which is not accepted or viewable for review on the Med+Proctor system. MedProctor cannot accept any HEIC files, encrypted or password-protected documents.

Once you have ensured your files are in a format that is compatible with the Med+Proctor site please upload them to your account, once your new upload has been reviewed the tag, "corrupt form," will be removed from your account.
