Help CenterMed+Proctor does not accept faxes:
Med+Proctor does not accept faxes from users, previous schools, or your healthcare practitioner. If you have a physical copy of your form that you would like to fax, you will need to scan your document or take a photo of your document and upload it to Med+Proctor. This can be done by using a scanner or a smartphone camera. Once the document is uploaded, it will be securely stored in your Med+Proctor account.

It allows users to easily upload all requirements for immunization compliance, such as immunization records, physicals, and other health documents.
