Your school assigned email address used during account registration will be your username. Your group and term should be located within your student portal. If you can't find it after checking there please contact your program coordinator or school helpdesk.  Your program coordinator or school helpdesk will be able to assist you in determining your correct group and term settings.  Students and Employees should upload documents to Med+Proctor for your school or workplace vaccine/immunization requirements. 

You will register using your school email address (the same one used to login to your school's student portal) to create your MedProctor account.

CWID is short for "Campus Wide ID", otherwise known as a student ID. This will be assigned to you by the school admissions or registrar office. If you do not have a student ID, please contact your school to have one issued. .
