How Do I Upload a Paper or Physical Copy of My Form?

You can take a picture of your form using your phone, email it to yourself, or scan it and upload it from your computer!

This way, you can have the convenience of accessing the form from any device, whether it be a laptop, tablet, or smartphone, and you can submit it quickly and securely without having to worry about any potential issues. Additionally, you can save the form to your device for future use, ensuring that you have a copy of the form on hand whenever you need it. 

Please note that Med+Proctor does not accept faxes. 

Once you have either scanned your document or taken a photo of it, you can upload it to Med+Proctor.

The types of formats that our system accepts are pdf, jpeg, or png files. You will be able to upload your form in the documents section of your Med+Proctor account.
