Your positive TB screening documentation

The current year’s chest X-ray report. If documentation of a completed course of prophylactic antibiotic therapy is provided, including medication name and dates of administration, then an older chest x-ray may be accepted.

The documented start and end date of your TB treatment, if submitted.
Please review the Positive TB Screening directions in the Tuberculosis (TB) Screening section of the HSIP Webpage. Submit all required documentation for approval.
Once approved, a Subsequent TB Symptom Screening tag will be created for you to complete a HSIP Tuberculosis Symptoms Screening form.
