The tag, "Blank Form," has been placed on your account to indicate that one or more of the records that have been uploaded did not go through correctly. This could be due to the wrong file being chosen, a document not being scanned correctly or a blank copy of an immunization certificate has been uploaded.
Please locate your immunization records and follow these directives to submit another document:
Go to your "status" page and click on "click here to submit another document" and you will be directed back to the document section where you can upload forms or download the optional immunization certificate
Once we receive and process updated documentation (that does not appear as blank) this tag will be removed.
