Help CenterThe tag, "COVID vaccine manufacturer missing," means that the vaccination record provided does not list the manufacturer of your covid vaccination. Your institution requires you to submit proof of your covid vaccination including the manufacturer and vaccine administration date. If you have documentation of your covid vaccinations that lists the vaccine manufacturer please upload it to your MedProctor account. If not you will need to work on obtaining a copy of your vaccination record that includes this information.
Common COVID vaccine manufacturers: AstraZeneca, BioNTech, Johnson & Johnson, Moderna, Novavax, Pfizer
How to submit another document:
- Sign into your MedProctor account
- Go to your "status" page
- Click on "click here to submit another document."
From there you will be directed back to the document section where you can upload forms or download the optional immunization certificate.
