Your COVID-19 vaccination documents did not include the required COVID-19 booster dose. In order to be compliant for the COVID-19 vaccination requirement, you must have your initial doses and a booster dose. CDC recommends a booster of either Pfizer-BioNTech or Moderna COVID-19 vaccine 5 months after the final dose in the primary series. Please submit your booster dose in order to be compliant.
