Missing PBA TB Clinical Assessment Form

You are required to upload the PBA TB clincal assessment form, which can be found in your Med+Proctor account in the "Document" tab to download. A TB test is required to be completed and your healthcare provider must fill out the details of the test on the PBA TB clincal assessment form. The form will then need to be uploaded to your Med+Proctor account.
