UW HSIP- Admin Review Name Difference

You have submitted a document under a different name that does not match your name in the UW Student Database. Please send documentation of your legal name change to HSIP at myshots@uw.edu so we can accept records under both names (e.g., marriage certificate or other legal name change document). Your prompt attention is appreciated. 

 HSIP will upload your legal name change documentation into your Med+Proctor account to accept any submissions with either name.
