Previous positive TB Screen – A new TBSS form must be completed annually in December.

School of Nursing – Students in the School of Nursing who reported any YES answers on the HSIP Annual TB Risk Assessment form must complete the TBSS form OR If you have a previous positive then BOTH the Annual TB Risk Assessment and HSIP TBSS form must be completed annually in December.
If you indicate “Yes” to one of the questions on the TBSS form, HSIP staff will follow up with you through email to clear your form.
