UW HSIP- Exemption Submitted

You have submitted an exemption request to one of your HSIP requirements. The HSIP exemption form must be completed and sent to HSIP via myshots@uw.edu, not through your Med+Proctor portal. Please send your completed HSIP exemption form to HSIP through email and we will forward your request to the designated reviewer.
