UW HSIP- HepB Chronic Infection Status

You have submitted documentation of two Hepatitis B vaccine series followed by a non-reactive HepB Surface Antibody Titer, the HSIP Chronic Hepatitis B Vaccine (HBV) Infection: Counseling Letter for Providers form, and the HSIP Student Acknowledgment form. We will inform your program of your status. You are required to inform your practicum site(s) of your HepB Chronic Infection status.
