Help CenterYou have submitted a record under a different last and/or first name that does not match your name listed in your Med+Proctor.
Med+Proctor does not have access to the UW Student Database, so they will mark any submissions that don't match your preferred name listed in Med+Proctor. HSIP will then check to see if your name on the record is in the UW Student Database and remove the tag, if it is listed.
If not, then the UW HSIP- Name Change Document tag will not be removed until you have submitted name change documentation.
Please submit a legal document (e.g., marriage certificate or other legal name change document) of your name change into Med+Proctor so we can accept records under both names.
