UW HSIP- Negative Childhood Immunization

You have attested "No" to the Childhood Immunization questionnaire in your account, indicating that you have not received childhood vaccinations for Tetanus, Diphtheria, Pertussis (DTaP) and/or Polio (IPV).  

  

Since the Childhood Immunization questionnaire is an attestation, you do not need to provide records of the DTaP and IPV vaccines. You can update your "No" response to "Yes" for the Childhood Immunization questionnaire in Med+Proctor if you or your adult guardian believes you received these vaccines in your childhood.   

  

If you and/or your adult guardian does not believe you received the DtaP vaccine series in your childhood, then you will need to complete a Tdap dose. Please submit one dose of Tdap completed within the last 10 years and upload the documentation to Med+Proctor. Once submitted, you can update your "No" response to "Yes" for the Childhood Immunization questionnaire in Med+Proctor 

  

Completing the full adult make-up series for DTaP and Polio is not required and not tracked by HSIP, but it is highly recommended to protect yourself from disease. The full adult make-up series for both vaccines is as follows: 

  

  1. Tetanus, Diphtheria, Pertussis - 3-dose series (Dose #2 & #3 can be the Td vaccines (without pertussis): 

  1. Dose #1 (should be a Tdap vaccine) is required by HSIP 

  1. Dose #2 should be completed 1-2 months after dose #1, and  

  1. Dose #3 should be completed 6-12 months after dose #2.  

  

  1. Polio - 3-dose series:  

  1. Dose #1 can be completed at anytime 

  1. Dose #2 should be completed 1 month after dose #1, and  

  1. Dose #3 should be completed 6-12 months after dose #2.  

  

Please email HSIP at myshots@uw.edu once you have updated your questionnaire response and we can remove the UW HSIP- Negative Childhood Immunization tag from your Med+Proctor account.
