Help CenterYou have attested "No" to the Childhood Immunization attestation, indicating that you have not received the childhood vaccination series for Diphtheria, Tetanus, Pertussis (DTap) and Polio (IPV). Since the Childhood Immunization item is an attestation, you do not need to provide documentation of the DTap and IPV vaccine series. You can attest "Yes" if you and/or your adult guardian believes you received these vaccines in your childhood.
If you and/or your adult guardian do not believe you received these vaccines in your childhood, then you will need to start the adult make-up. Please reach out to HSIP via myshots@uw.edu for next actions regarding completing the Childhood Immunizations requirement.
