UW HSIP- Rejected Lab on Imms Summary

You have submitted an HSIP-required titer and/or TB screening lab result that is on an immunization summary. HSIP requires the full lab report of your titer for approval.  

  • HSIP accepts MMR and Varicella IgG Antibody titers results from an Employee Health immunization summary but not from a clinic immunization summary.  

  • HSIP does not accept the HepB Surface Antibody titer and IGRA Blood Draw TB screening results from either an Employee Health immunization summary or clinic immunization summary. 

Resources on how to obtain your full lab report: 

If you submitted an Employee Health immunization summary with the titer result, then you will need to reach out to your employer/Employee Health Team for the full lab report. 

If the lab was completed through LabCorp or Quest Diagnostics, you may be able to find the lab report through either the LabCorp patient portalorQuest Diagnostics patient portal.     

Your lab report may be in your MyChart/patient portal, please see how to obtain the full lab report:  

  • If you have access to your records through a patient portal, you should be able to access your report by going to the laboratory details and clicking on the printer icon in the upper right-hand corner. Then save a copy of the print screen as a PDF. This document should have your lab results, the collection date, your full name, and the source of the record. 

  • If you are using the MyChart mobile app, then you will need to click on the three dots by "Test Results" at the top to obtain the full lab report.  

 

 

  • If you use a Kaiser Patient Portal, there is no printer icon in the upper right-hand corner in the lab tab details page. Instead, you will need to save a copy of the print screen as a PDF using your browser’s print function. You can do this by doing "Ctrl (Command on Mac) + P" or as shown below: 

 

 

 

If you cannot obtain the full lab report, then you will need to complete a new lab for that titer and/or TB screening and submit the lab report for approval.
