UW HSIP- Rejected Lab on Imms Summary

HSIP accepts MMR and Varicella IgG Antibody titers results from an Employee Health immunization summary but not from a clinic immunization summary.  

HSIP does not accept the HepB Surface Antibody titer and IGRA Blood Draw TB screening results from either an Employee Health immunization summary or clinic immunization summary. 

If you have access to your records through a patient portal, you should be able to access your report by going to the laboratory details and clicking on the printer icon in the upper right-hand corner. Then save a copy of the print screen as a PDF. This document should have your lab results, the collection date, your full name, and the source of the record. 

If you are using the MyChart mobile app, then you will need to click on the three dots by "Test Results" at the top to obtain the full lab report.  

If you use a Kaiser Patient Portal, there is no printer icon in the upper right-hand corner in the lab tab details page. Instead, you will need to save a copy of the print screen as a PDF using your browser’s print function. You can do this by doing "Ctrl (Command on Mac) + P" or as shown below:
