You have submitted a screenshot from your patient portal (such as MyChart), but the screenshot does not show your full name in your record, or the collection date of your blood draw.
How to print your immunization documents through your patient portal:
To obtain official immunization documentation, please return to your patient portal account and find your immunizations. At the top of the page with your immunizations, click on the printer icon. It will generate a PDF document that will show your name, date of birth, and immunizations. If you prefer to send only the immunizations requested, you can click ‘View Details’ on the required vaccinations and click on the printer icon there. Please save a copy of the print screen as a PDF and resubmit it to your account.
How to print your full lab report through your patient portal:
If you have access to your records through a patient portal, you should be able to access your report by going to the laboratory details and clicking on the printer icon in the upper right-hand corner. Then save a copy of the print screen as a PDF. This document should have your lab results, the collection date, your full name, and the source of the record.
If you are using the MyChart mobile app, then you will need to click on the three dots by "Test Results" at the top to obtain the full lab report.
If you use a Kaiser Patient Portal, there is no printer icon in the upper right-hand corner in the lab tab details page. Instead, you will need to save a copy of the print screen as a PDF using your browser's print function. You can do this by doing "Ctrl (Command on Mac) + P" or as shown below:
If the lab was completed through LabCorp or Quest Diagnostics, you can find the lab report through their sites which, respectively, are https://www.labcorp.com/patients/results and https://myquest.questdiagnostics.com/web/home.
Here is a screenshot version of the instructions, highlighting the correct button to press. Please resubmit the print screen PDF of the lab report for approval.
