The date read of your PPD skin test(s) AND/OR

The results in millimeters (MM) of your PPD skin test(s)

The HSIP TB PPD Skin Test Form is recommended to ensure all required documentation is included in submission. Please have your provider either update your documentation to include this information or have them fill out the HSIP TB PPD Skin Test form to ensure all documentation components are included. Resubmit the revised documentation or the completed HSIP TB PPD Skin Test Form for approval.
