You submitted a blank HSIP Provider Verified Immunization Record form.

You submitted a HSIP Provider Verified Immunization Record form that was signed by yourself or a non-medical provider.

You submitted an immunization booklet that does not include a provider’s stamp or signature by your HSIP-required vaccines.

You submitted a document that is password protected.
Please resubmit with a healthcare provider's signature or submit other acceptable records. Official immunization records such as patient portal documents, state immunization documents, or pharmacy vaccine records are acceptable without a signature.
Your dose(s) may be in the State Immunization Registry of the state you completed your vaccines in. Please use the HSIP Map of State Immunization Resources and click on the state you completed the vaccines from to find how to obtain your records.
