UW HSIP- Rejected Unverifiable Dose(s)

You have submitted a document with dose(s) that are unverifiable because of the following reasons: 

  • The immunization booklet does not include a provider’s stamp or signature by your HSIP-required vaccines.  

  • The written immunization record does not include a dated provider's signature. 

  • The K-12 immunization record does not include a dated provider's signature.  

Please resubmit with a healthcare provider's dated signature or submit other acceptable records. Official immunization records such as patient portal documents, state immunization documents, or pharmacy vaccine records are acceptable without a signature.  

Your dose(s) may be in the State Immunization Registry of the state you completed your vaccines in. Please use the Looking for State Immunization Records? page and click on the state you completed the vaccines from to find how to obtain your records. You can find more information, including locations to receive vaccines or titers, on the HSIP webpage.  

If you are unable to find your records in a verifiable source, then HSIP highly recommends having your current provider or a local public health clinic document your historical vaccine dates in an official medical record or state immunization registry. Once entered in a verifiable source, please submit the record for approval.
