Help CenterA childhood immunization attestation must be completed upon entry into your program. This is a questionnaire to see if you have previously received the childhood vaccination series for Diphtheria, Tetanus, Pertussis (DTaP) and Polio (IPV). Since the Childhood Immunization item is an attestation, you do not need to provide documentation of the DTap and IPV vaccine series.
If your response is yes, you have had the DTaP and IPV vaccines in your childhood, then click ‘Yes’ and continue onto the next requirements. No further action is required for this requirement.
If you and your parent/guardian do not think you received the DTaP or IPV vaccines in your childhood, please click ‘no’, and contact myshots@uw.edu to discuss follow-up with HSIP staff.
