UW HSIP - Required Childhood Attestation

A Childhood Immunization attestation must be completed upon entry into your program. This is a questionnaire in your Med+Proctor account to confirm if you have previously received the childhood vaccination series for Diphtheria, Tetanus, Pertussis (DTaP) and Polio (IPV).  Since the Childhood Immunization item is an attestation, you do not need to provide documentation of the DTap and IPV vaccine series. 

 

Please complete the Childhood Immunization questionnaire in your Med+Proctor account by going to the Status page and clicking on "Questionnaires" in the sidebar. 

 

  • If your response is Yes, you have had the DTaP and IPV vaccines in the past, then click ‘Yes’ and continue onto the next requirements. No further action is required for this requirement. 

 

  • If your response is No, you have not had the DTaP and IPV vaccines in the past, then click ‘No’. Your account will be tagged with UW HSIP- Negative Childhood Immunization which will explain the next action for the Childhood Immunization requirement.  

 

Important: Once admitted into an HSIP-participating program, you have an initial registration block. Regardless of your ‘Yes’ or ‘No’ answer to the Childhood Immunization attestation, your initial registration block will be lifted in the next 24 hours after the UW HSIP - Required Childhood Immunization tag is removed. Please make sure to complete the questionnaire as soon as possible.
