UW HSIP - Required COVID-19 Vaccine

Submit documentation of an updated COVID-19 vaccine for the current FALL season (beginning in August).

·        Pfizer or Moderna vaccine for the current FALL season

OR

·        Novavax vaccine dated for the current FALL season AND one additional prior COVID-19 vaccine dose.

 
Annual vaccination required. List of due dates by program are available on the HSIP website.

RECENT COVID-19 Infection: Individuals who have tested positive for COVID-19 infection in the past 90 days, complete a COVID-19 extension form.

NOTE:

·        Full student name and full date of vaccine must be included in documentation.

·        Vaccine manufacturer must be included in documentation.

·        Document must be from a verifiable source (medical record, clinic or provider signature/stamp, certified state immunization registry, or lot #’s provided).
