BOTH STEP 1 AND STEP 2 ARE REQUIRED.
A titer will NOT be accepted until after a complete HepB vaccine series has been submitted. If you cannot obtain documentation of a previous vaccine series, you are required to repeat the vaccine series. If your vaccine series is in process, submit where you are in the series. New requirement tags will be created for you to complete the vaccine series and a post-vaccination titer.
STEP 1: Hepatitis B Vaccination Series (DOCUMENTED SERIES REQUIRED)
- Documentation of 3 Hepatitis B vaccine doses (Engerix-B, Recombivax HB, Hepatitis AB/Twinrix, or other HepB vaccine) administered by acceptable CDC spacing guidelines
OR

Documentation of 2 Heplisav B vaccine doses administered at least 28 days apart
AND
STEP 2: Post-vaccination QUANTITATIVE HEPATITIS B SURFACE ANTIBODY Titer (HBsAb-QN or Anti HBs Concentration)

LAB REPORT with NUMERIC interpretive reference range required. An immunization summary does not constitute a lab report.

Negative/Equivocal results may be accepted without a numeric reference range. Additional vaccination and repeat titer required following a negative titer.
If your titer is NEGATIVE or EQUIVOCAL, submit the negative titer and new requirement tags will be created for you to:

Receive and submit one additional vaccine dose (Heplisav-B highly preferred) due immediately

Get a repeat titer 1 month after the booster dose. You do NOT need to complete a full second HepB vaccine series unless the repeat titer is also negative.

If your repeat titer is negative or equivocal, new requirement tags will be created for you to receive the remainder of the second vaccination series.

Get a final titer 1 month after the last vaccine dose in the second series.

Full student name and full date of blood draw or vaccine must be included on documentation.

Documents must be from a verifiable source (medical record, clinic or provider signature/stamp, or certified state immunization registry. Handwritten K-12 records signed by a parent are not acceptable.)
