Submit documentation of an Influenza (Flu) vaccine administered during the current flu season (beginning in August).
· Only Influenza Vaccines administered after 08/01 of the current flu season will be accepted.
· Vaccines not administered within the current flu season will be rejected.
Annual vaccination required. List of due dates by program are available on the HSIP website.
NOTE:
· Full student name and full date of vaccine must be included in documentation.
Document must be from a verifiable source (Medical record, clinic or provider signature/stamp, or certified state immunization registry). Handwritten K-12 records signed by a parent are not acceptable.
