UW HSIP- Subsequent HepB Carrier Records

You have submitted documentation indicating you are a Hepatitis B Carrier. You will need to submit the following documentation to document your status:

·         Obtain your most recent Hepatitis B Surface Antigen (HBsAg) titer lab report.

·         Please have your provider complete the HSIP Acute-Chronic Infection Counseling Form.

Once you have completed the form, please submit them directly to HSIP at myshots@uw.edu.

Please note that your status as a HepB Carrier will be shared with your program. Students are expected to inform their practicum of their HepB Carrier status as well.
