Help CenterYou have submitted record(s) indicating that you have tested positive for Tuberculosis (TB), you are required to submit the following to document your positive TB screening:

Your positive TB screening documentation

Positive IGRA Blood Draw lab report OR

10 mm or greater PPD skin test record

A chest x-ray report AFTER your positive TB screening.

If submitted, the documented start and end date of your TB prophylactic antibiotic therapy treatment, medication name and dates of administration are required.
For more information, please review the directions in the Tuberculosis (TB) Screening section of the HSIP Webpage. Submit all required documentation into Med+Proctor for approval.
Once approved, a Subsequent TB Symptom Screening tag will be created for you to complete a HSIP Tuberculosis Symptom Screening (TBSS) form.
