UW HSIP- Subsequent Pos TB Screening

You have submitted record(s) indicating that you have tested positive for Tuberculosis (TB), you are required to submit the following to document your positive TB screening:   

  • Your positive TB screening documentation  

  • Positive IGRA Blood Draw lab report OR 

  • 10 mm or greater PPD skin test record 

  • A chest x-ray report following your positive TB screening.    

  • If submitted, the documented start and end date of your TB prophylactic antibiotic therapy treatment, medication name and dates of administration; a chest x-ray POST-treatment is required.   

   

For more information, please review the directions in the Tuberculosis (TB) Screening section of the HSIP Webpage. Submit all required documentation into Med+Proctor for approval.  

 

Once approved, a Subsequent TB Symptom Screening tag will be created for you to complete a HSIP Tuberculosis Symptom Screening (TBSS) form.
