UW HSIP- Subsequent Pos TB Sx Screen

Please complete and submit the HSIP TB Symptoms Screening (TBSS) Form into your Med+Proctor for approval.   

If you indicate “Yes” to one of the questions on the TBSS form, HSIP staff will follow up with you through email to clear your form.
