Help CenterHSIP has been informed by your HSIP-participating program that you have withdrawn. We have archived your Med+Proctor account from your program’s requirement group. If you believe you have been archived incorrectly, please reach out to HSIP at myshots@uw.edu immediately.
If you have uploaded records, you will still have access to the records you have in your Med+Proctor account.
If you have been reinstated into your HSIP-participating program, you and/or your program will need to inform HSIP at myshots@uw.edu. We will move your account back to your HSIP-participating program’s requirement group. We will also inform you of the requirement(s) you need to complete before you are able to register for the quarter you return.
