UW – Needs 1 MMR (Measles/Mumps Under 1) 

You received a Measles and/or Mumps vaccine, or MMR (Measles, Mumps, Rubella) vaccine, before you were 1 year old, which will not fulfill your requirement. You are required to show proof of two valid Measles and Mumps vaccines, or positive results of both Measles and Mumps IgG titers (blood immunity tests).    

 Please submit proof of another valid MMR vaccine, or positive Measles and Mumps titer test results. 

NOTE: If one or both of your Measles and Mumps test results is equivocal or negative, you will need to receive a second MMR vaccine to complete the requirement.
