UW Param- Admin Review Name Difference

You have submitted a record under a different last and/or first name that does not match your name or your preferred name in the UW Student Database. Since there is a name change, please submit legal document (e.g., marriage certificate or other legal name change document) of your name change onto Med+Proctor so we can accept records under both names. 

 You currently have the UW HSIP- Admin Review Name Difference tag which will not be removed until you have submitted name change documentation.
