Your positive TB screening documentation

A chest x-ray report following your positive TB screening.

If submitted, the documented start and end date of your TB prophylactic antibiotic therapy treatment, medication name and dates of administration, AND a chest x-ray post-treatment is required.
Please review the Positive TB Screening directions in the Tuberculosis (TB) Screening section of the HSIP Webpage. Submit all required documentation for approval.
Once approved, a Subsequent TB Symptom Screening tag will be created for you to complete a HSIP Tuberculosis Symptoms Screening form.
