Tetanus, Diphtheria, & Pertussis (Tdap) vaccination administered in the past 10 years.
OR

Tetanus & Diphtheria (Td) vaccine administered in the past 10 years AND documentation of one Tdap vaccine administered on or after 6/1/2005.
NOTE:

Tdap/Td vaccines expire every 10 years. An additional Tdap/Td vaccine will be required

Full student name and full date of vaccine must be included in documentation.

Document must be from a verifiable source (medical record, clinic or provider signature/stamp, or certified state immunization registry record). Handwritten K-12 records signed by a parent are not acceptable.
