Why did I receive notice that I am missing Health Professional Information?

Health Professional Information includes 2 of the 3: Physician/Health Provider Signature, NPI number, and/or office stamp. 

If your University requires both immunization dates and a tb test the Health Professional Information needs to be under BOTH sections. One is not sufficient.

Students and Employees should upload documents to the medproctor.com vaccine & immunization software for your school or workplace requirements.
